Building a form
To build a form, please click:
- Manage
- Forms, then
- New form
First, use the Details tab to save the overall form settings, then click Save
Next, create your form items, using the Items Tab. The items are the questions and information you add to the form.
Click Add form item
Here you can create all the items that will appear inside your form, including:
A Section which breaks up your form into parts
A Paragraph which is a chunk of text that appears inside your form
A Field which allow you to collect data in various formats:
- Text
- Long text
- Select one (dropdown/Radio list) - select only one from a list of options
- Select multiple (checkbox list) - select multiple options offered in a list
- Checkbox
- Yes / No
- Number
- Date
- Signature
- Annotation
Once you've entered the settings for the item click Add, which will add it to the form
Your form items show in the order they're added, and you can reorder, editor delete them
Click Preview so see how your form looks
Click Save when you're happy with your form items
If you wish to require customers provide their consent (e.g. to store their personal information in your account) before they're able to submit it, you can do so on the Consent tab.
Automating a form
Once you have built your form, you can have the system send the form request email automatically for you, asking customers to complete it. This is done by linking a form to a service and setting when the form request email should be sent when that service is booked.
Completing a form
Forms request emails related to an appointment can be automatically sent to customers. Forms can also be manually requested and completed in customer profiles.
